Format

| SlotNumber | Integer (8 bits) | Slot number associated with the memory space of the application to be created. |
| SlotVersion | Integer (8 bits) | Slot version associated with the memory space of the application to be created. |
| QuotaLimit | Integer (16 bits) | Maximal memory consumption of the application in 32 byte blocks. |
| EncApplicationKey | Raw data (length 32 Bytes) | The encrypted initial application key for the DAM application. |
| DamMac | Raw data (length 8 Bytes) | The MAC calculated by the card issuer to allow delegated application creation. |
